1. Request for Copying Services
2. Request for Textbooks and Teaching Materials
3. Request for Classroom Supplies and/or AV Classroom Set‐Up

Please submit completed forms to the MICP office at micp@georgetown.edu or via fax at 202.687.4881.

If you would like to email one of the forms to us, please save the form to your desktop first and complete it afterward. If you prefer to fax any of the forms to us, you can complete them online and print them without having to save them. Thank you!
